Use the Matrix field to capture related answers in a simple grid layout field.
The Matrix field supports both a single answer per row, or multiple via checkboxes.
After adding the Matrix field and making any tweaks, you can press Live Preview or Publish to test it out:
For multiple answers per row, simply check the “Multi-Select” toggle and try it out:
Optionally, you can add Required Smart Validation to ensure every answer in the field is always filled out:
